Physical Therapist – Home Care Services

Macas Home Care LLC, a Residential Service Agency in Maryland, is seeking a motivated and experienced Physical Therapist (PT) to join our team and contribute to our mission of providing exceptional home care services throughout the greater Maryland area.

This position will support clients primarily in Maryland and surrounding locations.

Position Overview
The Physical Therapist will be responsible for evaluating and treating patients in their homes, developing individualized therapy plans, and helping clients improve mobility, strength, balance, and overall functional independence. This role requires excellent clinical skills, strong communication abilities, and a commitment to providing high-quality, patient-centered care.

Key Responsibilities:
  • Conduct comprehensive physical therapy evaluations and assess patients' functional abilities, mobility, strength, balance, and rehabilitation needs.
  • Develop individualized treatment plans based on each patient's condition, goals, and physician's orders.
  • Implement evidence-based therapeutic interventions to improve mobility, reduce pain, prevent disability, and promote independence.
  • Educate patients, family members, and caregivers on exercises, mobility techniques, fall prevention, and home safety.
  • Monitor patient progress, document treatment outcomes, and modify care plans as needed.
  • Communicate patient progress and any significant changes to physicians, family members, and other members of the healthcare team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Collaborate with interdisciplinary team members to ensure coordinated, high-quality patient care.
  • Maintain open communication with Clinical Leadership and the Office Administrator regarding patient status and any concerns.
  • Participate in team meetings and quality improvement initiatives.
  • Perform other duties as assigned by the Administrator.

Requirements:
  • Active Maryland Physical Therapist (PT) licenseRequired.
  • License allowing practice in both Maryland (MD) and Washington, D.C.Preferred.
  • Minimum of 6 months of physical therapy experience (home health experience preferred but not required).
  • Minimum of a Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program.
  • Strong clinical assessment and rehabilitation skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
  • Reliable transportation for home visits (mileage reimbursement provided).
  • Proficiency with computers, tablets, or iPads for documentation and communication.
